Subject: Empanelment of Private HCOs under CGHS Patna under continuous empanelment scheme, 2017 & 2022.
With reference to the Directorate’s (DGHS) Office Memorandum No. S.11045/ 36/2012/ CGHS/HEC dated 16-10-2015, 26-12-2017, latest memorandum No. S.11011/ 09/2022/ CGHS(HEC) dated 13-09-2022, Corrigendum order dated 16-09-2022 and as per terms & condition mentioned in tender documents and Memorandum of Agreement signed between the undersigned and HCO, the following HCO is hereby empanelled under CGHS, Patna for a period of Two years from the date of issue of this Office Memorandum or till next new empanelment which ever is earlier :-
