SUB: Notification for continuous empanelment for CGHS Gwalior under the administrative control of Additional Director CGHS Kanpur under continuous empanelment Scheme 2022.
In compliance of the Ministry of Health & F.W., Nirman Bhawan, New Delhi O.M. No. S. 11011/ 9/ 2022-CGHS (HEC) dated 13th September 2022 and subsequent CGHS. Hospital empanelment Committee recommendations on the subject cited above, Paridhi Hospital and Care, 111,112, Kailash Vihar City Center Gwalior has been approved for CGHS, Gwalior under the Administrative control of Additional Director CGHS Kanpur. On the basis of response received form the hospital and acceptance of terms and conditions for empanelment, MoA is signed between Additional Director CGHS Kanpur and the above hospital.
The empanelment shall be for a period of two years from the date of issuance of this O.M. for providing medical facilities to the CGHS beneficiaries as per CGHS rate list 2014 (updated on 14.05.2021) and according to the terms & conditions laid down in MOA signed by the authorized signatory of the above.
A copy of this office order along with the list of clinical services laboratory and diagnostic services shall be placed on the website at cghs.gov.in
