Sub: – De-empanelment of Vinayak Netralaya, Indore from the panel of CGHS Indore.
On surprise checking/inspection conducted by myself alongwith Dr. K. K. Sharma, SMO CGHS Bhopal on 10.03.2023 at 03:15 PM, it is found that the Vinayak Netrala, located at 203, Lakshya Badgara Crest Tower, Near Janjirwala Square, Above Lari Sarees Indore (M.P) – 452001 is neither in existence and nor found functioning and has been sold-out to another party without informing to CGHS. Therefore empanelment of Vinayak Netralaya, Indore is being discontinued from the panel of Central Govt. Health Scheme, Indore w.e.f 14.03.2023.
Performance Bank Guarantee of Rs. 2,00,000/- (Rs. Two Lakh only) submitted by the hospital will be forfeited/released subjected to deduction of penalties and adjustment/recovery of any outstanding dues (if any) after three months or settlement of all the bill, whichever is the latter, from the date of de-empanelment of the hospital as per the rules/MOA.
