Subject: Continuous Empanelment Scheme 2022 for empanelment of private Health Care Organizations (HCOs) under CGHS- corrigendum
With reference to the above subject, attention is drawn to Office Memorandum No S S-11011/ 09/2022/CGHS(HEC) dated the 13th September, 2022 vide which orders for continuous empanelment scheme 2022were issued and to state that there were inadvertent typographical errors in Clause 2,3 & 17 in the enclosed application form and MoA for empanelment under CGHS 2022. The corrected Clauses 2,3 & 3 should read as under:
Clause 2. DEPOSITION OF FEE FOR APPLICAION AND EMD
Application document fee of Rs. 1000/- (non-refundable) and Earnest Money of Rs. 100,000/-(One lac) in the form of Demand Draft from any Scheduled Bank, payable to ‘Pay & Accounts Officer, CGHS Delhi, in case of Delhi and NCR / Concerned Additional Director in case of other cities has to be submitted by the new HGO’s. Physical DD shall be delivered along with the Application form.
Clause 3. SUBMISSION OF APPLICATION FORM BY NEW HGO’s
Completed and duly signed application form along with enclosures (in duplicate) shall be placed in closed envelop. The requisite Demand Draft for Rs.1000/ and 1, 00,000/- shall be enclosed in another closed cover and titled. Both the envelops shall be enclosed in a sealed cover with address and contact number of the HCO and shall be delivered at the Office of Additional Director HQ CGHS Delhi CGHS Bhawan Sector-13; R. K Puram New Delhi110066, in case of Delhi & NCR and at the office of Additional Director CGHS of concerned City in other cities.
Clause 17. Last para of Clause 17. PROCESSING OF CLAIMS/BILLS BY THE BCA
The payment of the bill/claim to the Private Hospital concerned will be made directly by CGHS in respect of CGHS pensioner beneficiaries, etc.
The other terms and conditions of Application for shall remain unchanged. The Application form with corrections is attached herewith.

Subject: Continuous Empanelment Scheme 2022 for empanelment of private Health Care Organizations (HCOs) under CGHS.
With reference to the above subject, attention is drawn to Office Memorandum No S-110045/36/ 2012 dated the 26th December, 2017 vide which orders for continuous empanelment scheme were issued. In this regard the undersigned is directed to state that the Ministry has reviewed the Application form along with the Draft for Memorandum of Agreement(MoA) and revised the same.
The revised Application Form for Continuous Empanelment Scheme 2022 along with revised MoA are enclosed. CGHS package rates as prevalent and as revised from time to time shall be applicable. Private Health Care Organizations (HCOs) seeking empanelment under CGHS Cities can apply to the Additional Director, CGHS of concerned City in this regard. The empanelment shall be for a period of two years.
The HCOs already on CGHS panel shall submit their Letter of acceptance for the revised MOA within 15 days of issue of this OM to the Additional Director of concerned City for continuation on CGHS panel for another two years i.e., till 30th September 2024, failing which it shall be deemed that they are not interested in continuation under CGHS and shall be removed from CGHS panel. They shall sign new revised MOA and submit Valid Performance Guarantee (for 30 months) within 3 months of issue of this OM.
